Good night Beth! See ya MONDAY!!! hehehe

Nancy if Kristina was that high when she was due for her shot of regular, I'd double the amount she normally gets! But that would only be 2 extra units!!

Nancy, you said he doesn't usually take a nightime dose; does that mean he takes one if he's above a certain #? If so, how much?

laura, when Adam woke up this a.m. he was 86 so I really didn't expect this to happen. What percentage to you all think would be appropriate at 531?
Thursday, February 20, 1997 at 22:26:50 (EST) - cco-fl4-12.ix.netcom.com

laura from fl:
Hannah just started using H at dinner and bedtime if she's over 180 about a month ago; the endo prefers that for her (now anyway) so I don't have to worry about lows 6 hours later.
Thursday, February 20, 1997 at 22:26:26 (EST) - 76.st-louis-008.mo.dial-access.att.net

Beth :
Nancy - even with Sara's lows we've not dealt with rebound sugars. Yeh, it does sound pretty high for a rebound.
Thursday, February 20, 1997 at 22:25:50 (EST) - tty37.pld.com

Melissa :
Thanks Moys! Good to see you!!
Thursday, February 20, 1997 at 22:25:08 (EST) - 207.22.198.47

Debbie :
Nancy, I think that anytime there are ketones in the urine you need more insulin. If you have humalog that would be good to use since it wont last so long.
Thursday, February 20, 1997 at 22:25:04 (EST) - ppp-10.ts-3.nyc.idt.net

Nancy :
laura/Beth, his hypo yesterday wasn't what I would consider severe, but since he has started on the H the hypos have really decreased. He was 52, but it took me 2 hours to get him to 106. It really does sound like a rebound, but it is rather high though.
Thursday, February 20, 1997 at 22:24:27 (EST) - cco-fl4-12.ix.netcom.com

Debbie :
Yes, I gave him extra R with his am shot plus two extra units of R at Lunch. At dinner he had his regular shot. Today his numbers were great: 99,128 and 251! Go figure. He went to school today.
Thursday, February 20, 1997 at 22:23:40 (EST) - ppp-10.ts-3.nyc.idt.net

laura from fl:
510 is up there Nancy, I think a little extra insulin would help.

Goodnight. Your welcome.
Thursday, February 20, 1997 at 22:22:52 (EST) - will_34s.citynet.net

laura, he doesn't ususally take a bedtime injection and yes, we do have Humalog. He recently started using it, however from this forum tonight it sounds as if Regular is what we need at this point.

Could the highs be as a result of his hypo yesterday?
Thursday, February 20, 1997 at 22:21:58 (EST) - 76.st-louis-008.mo.dial-access.att.net

Nancy :
Debbie, did you give TJ any extra insulin?
Thursday, February 20, 1997 at 22:21:10 (EST) - cco-fl4-12.ix.netcom.com

laura from fl:
Nancy, maybe you missed my other question. Does he normally get insulin at this hour? How much? and a new one: Do you have H on hand?
Thursday, February 20, 1997 at 22:20:14 (EST) - 76.st-louis-008.mo.dial-access.att.net

Beth :
Middle of nowhere???? Why we're centrally located I'll have you know. :)
Thursday, February 20, 1997 at 22:20:04 (EST) - tty37.pld.com

Larry:
Joyce- That question puts me in the mind of that one Senfield episode where Elaine tries to get her records changed because they have her labeled as a difficult patient. The more noise you make about it the more trouble you might cause. As a pyhsician, we are always taught that changing a medical record in any way to remove or delete information is considered medical legally fraud. A lot of juries have awarded millions of dollars to plaintiffs because of altered medical records. The most you could ask for is for the doc to add some information to a new note that would better explain or clarify some previous info. This may help. You need to be able to talk to and trust your doctor, though.
Thursday, February 20, 1997 at 22:20:04 (EST) - will_34s.citynet.net

Debbie :
Sounds like TJ yesterday. He woke up at 384 with moderate ketones but no signs of illness. I took him to the doctor anyway but he didn't find anything. His BS didn't come down till bedtime!
Thursday, February 20, 1997 at 22:19:54 (EST) - ppp-10.ts-3.nyc.idt.net

Nancy :
Last night he had a hypo so he was rather wiped out today. I could get him going this a.m. so he stayed home today. His activity level hasn't been as high. Boy, I sure am stumped. He does not usually take a bedtime injection.

Larry, one more quick question, Can I make changes to the content of the records.. I have concerns about the content of my last visit to the allergist.. I am hardly a negligent parent.. and cringe to have it stated as such..or maybe that could just be another one of the "badges" i.e. battle scars a parent carries.. laugh.

You're daughter is participating in a research project that could actually help her someday!! What right does anyone have to make assumptions about her because of it??

I agree with you Nancy...I have 4 kids...and a whole lot going on... it sure would be nice if some things in life were easy... but I must admit... I do find myself looking on the bright side of things.. more than other people..

Well Larry, you sure sound like a great doc and dad also. Thanks for all your great input tonight!!!

Larry:
Joyce- Exact wording may differ from state to state but the actual records are the property of the clinic or doctor but you have a legal right to review them and have them sent to another physician. Requests for copies might cost you for the actual copying costs.
Thursday, February 20, 1997 at 22:11:16 (EST) - will_34s.citynet.net

Nancy :
Joyce, I thought that I would carry the records from one office to the other and remove the letter, but my conscience just wouldn't allow it. Just the knowledge of having two or more children is enough stress never mind worrying about the insurance side of it.

Good night SFN!! Thanks for coming!!
Thursday, February 20, 1997 at 22:08:10 (EST) - 207.22.198.47

Larry:
A doctor can put anything in a medical record that pertains to a patient but what happens to that information is what the doctor legally is responsible for. I would ask the doctor not to let that part of the chart to be copied and released and see what he says. Records cannot be legally released without your permission but you sign releases of information when you sign up for insurance coverage.

LOL!! So someone who's got alot of fat in their diet should be called "pre-heart attack"!

I know what you mean Joyce! I see a real difference in my family doctor depending on whether we see him first thing in the morning or at the end of the day!

why would a doctor feel obligated to put some information im a patients record about a condition which does not exist, hypothetically, could they be medically negiligent for reporting someone as "prediabetic" does that condition exisit in the DSMIII or whatever used to diagnosis?

Joyce, I was really upset so I call the Research Coordinator and she assured me that if this did become an issue with an insurance company that they would become involved and do what they could to interven and hopefully the insurance company would gain a new outlook on exactly what this research is all about.

I have to be honest- I don't know if you can get her records changed. If a patient asked me not to report something to an insurance company I knew was true, I would have to consider the consequence of hiding pertinent info which is called insurance fraud. On the other hand, if someone doesn't have a disease or illness and only is genetically predisposed, it is unethical, I feel, for an insurance company not to insure them. Being involved in a scientific study does not put you at a high risk of illness. I would think, actually, you would be a lower risk wanting to prevent illness. Who knows?

Yea it does SFN! So that's my dilema! the next time she gets sick, should I use the routine we always use (which usually results in a very high blood sugar before we're done) Or should I chance the NPH and just reduce the R???

Nancy, I got a great letter from someone that I posted in the message board! They have switched to H but keep R on hand for sick days! They switch to the R when their son is throwing up cause it takes longer to start working!! I guess it gives her more time to get something in him!! Then they treat highs with H! Then go back to the H when he's better!

Laurie, I've seen some things about that watch! I think one company is sueing the other company over it saying that someone messed up the testing on it and fudged the numbers!

Nancy... that is awful.. I think you can have it removed.. someone told me that in cases of mental health.. when someone is admitted or seeks treatment for mental health reasons, that can be removed.. maybe I got wrong info.. YAH, IS PREDIABETIC A NEW MEDICAL CONDITION??? you would have some legal grounds there i would think..

Melissa...you know that doctors are only human.. and in their minds (or so I think) they only view the medical part of a kid.. the here and now.. and in my case.. they were concerned about their liability... and job and own mental health.. Joyce, I mistakenly gave a copy of the letter that confirmed our daughter was ICA positive and our Peditrician put it in her file and said it was now a permanent part of her file and it was also illegal to remove it. I have now changed peditricians and we have resolved a few things. Some insurance companies view it as a pre-diabetic. What is a pre-diabetic, I have never heard of a pre-cancer etc.!

I do not know if Accolate affects blood sugars. I do not think so but I don't have my information here at home to check. I have not used it in a diabetic yet. I'll certainly be checking tomorrow.

Larry and Melissa, I agree with you.. we recently changed insurances and thus the new doctors... I really do not think their is a physican I can trust.. despite requesting them not to put info in Erin's chart about her involvement with the DPT it somehow is now part of her medical record.. I was told I could have that changed, Is that so?

If your child wakes up with very high blood sugar and ketones in the urine; but does not appear to be sick, should you keep them home from school?
Thursday, February 20, 1997 at 21:49:49 (EST) - ppp-10.ts-3.nyc.idt.net

Melissa :
SFN, once she can keep food down, then the NPH will cover a longer period of time so she won't have to have so many shots!! But if she can't keep anything down, then it's good to know that she won't have that peak of NPH to fight off later on in the day! When she's sick, I give the R when she needs it! Once she's gotten 4 shots of R in a day and she's keeping food down, then we'll go back to the old routine of 3 shots a day including NPH.
Thursday, February 20, 1997 at 21:49:12 (EST) - 207.22.198.47

Larry:
I agree with Melissa. What keeps you going back to a doctor who doesn't listen to you, take any suggestions, or even seem to give a darn about your kid's health? Get a new physician you can trust and talk to. There are a lot out there!
Thursday, February 20, 1997 at 21:47:19 (EST) - will_34s.citynet.net

Nancy :
Joyce, I can't understand why a doctor would respond like that!!!:-( If it weren't for research we wouldn't be where we are at today in medical science.
Sounds very insensitive to me, and I really hate to hear about a reaction like this. This mentality could really affect how our research dollars are spent!!!!

The medicine Accolate is not a steroid or any relation to steroids, it is a pill that prevents the airways from reacting to whatever triggers asthma in a person whether it is from allergies, exercise or stress. Most common side effect has been headaches which have required medicine but this has only lasted about 2-3 weeks. Of course, only your physician can prescribe medicine. ( Don't you hate disclaimers?)

Sorry, I got booted! Beth I'm glad that things are better with Sara....Joyce, how old is your daughter? How do you control the asthma? TJ takes Intal via nebulizer and Proventil as needed when he is having trouble. The worst times are when he has a cold and the asthma is really hard to control; which makes him throw up; which knocks the diabetes control for a loop! We have had to make a few emergency room visits, but no admissions since diagnisis.

POM..... hello.... just lurking for a bit.....I think that even though your advice was given years ago it is still good... when a diabetic is hospitalized we check blood sugars as often as "needed"... and treat with REGULAR only...when they get better we start up the NPH again.... of course all of this with MD orders...Hello ron and others.... just gonna sit back for a while.... I am LEARNING.......

Those glucose tablets just don't seem strong enough to me! Kristina has to eat 4 or 5 of them just to get 15 carbs! I think juice works so much better, under normal circumstances!

Stacy, I've never heard of those popcycles but they sound wonderful!!

Perfect Joyce! Also makes you more aware of what is going on with their bodies. The slightest rise in sugar levels makes you go in a tail spin wondering why. There have been highs we have never been able to explain.

She's doing fine. No more lows. No highs. Finally some rest!!

Joyce, I find that if a parent seems to be knowledgable that it is taken the wrong way at times. I feel it is our responsibility as a parent to arm ourselves with as much knowledge as we can, an afterall we are the ones that deal with the day-to-day stress of this illness. We only see our endo once every 3 months for approx. 20 minutes. It sure is hard to get it all in in that amount of time!!! I'm with you!!!!!

Larry, Yes, I will keep asking questions.. except it is difficult when as a parent who has dealt with asthma for almost 10 years, when I am honest with my doctor.. looking for options... I am threatened with being "reported"... I am discounted as a knowledgable and able parent...You are right though, having a diabetic of your own provided you with a special sensitivity which obviouslly is lacking in the allergist and pediatrician we dealt with last week.

Joyce- I agree that the medical profession is not always as sympathetic as they should be. I have a seven year old diabetic child and, although I have treated diabetics for years, I learned a totally different dimension to caring for a diabetic child having one of my own. No matter who you are dealing with, though, you need to stand up for what you feel is right. Keep asking questions.

We were told a few years ago to completely stop using the NPH when Kristina was really sick and couldn't keep anything down!! Then we wouldn't have to fight that NPH during the night or something!! It meant more shots but we were told to give her a shot whenever she went above 250! By the time she was getting 4 shots in one day, we knew it was ok to go back to using NPH!

Larry, yes she is not to be on steriods for the DPT, prior to some of the testing.. yes we are getting into medical ethics here.. but what upsets me is having to deal with a medical community who refused to see my daughter as a potential diabetic... yes, I understand their concern for the present.. but as a mother, I am in it for the long haul... it was an awful situation to be put in.. I wanted to know what their stake in her was.. and they made me the unreasonable one.. such is the parent dilemna..

Our doc is quite adamant, that we make decisions based on the Ketones. If Ketones, more Insulin. He gave a sliding scale for different blood sugars with Ketones, etc.

Joyce- Were you told not to have your daughter take steroids with this DPT-1? Also, there are other medications other than steroids that can be effective for an asthma attack, although steroids can be effective. Heck, what if she were allergic to steroids?

Joyce, which one of your other children has diabetes. We were told that we are to treat our daughter as if we had no knowledge of her risk of developing IDDM. I don't think that the steroids would affect the results of the DPT, but if you have any questions contact the research center. Do you have a Diabetes Research Coordinator?

Melissa :
If you click on "Sick Day Rules for Diabetes", they give an indepth explanation at how to increase the insulin! I usually increase by 1 or 2 units depending on how high she is! But I always just took my best guess! I was pleased with this article to find out I had been doing it right all along!!

I felt I was the victim of circumstances which I could have prevented if I would not have taken my daughter to a routine visit to her allergist... she was wheezing.. quite badly.. but this fluctuates... but she had been quite wheezy through the weekend... and the doctor said he would report me to CPS if I did not agree to giving her oral steriods.. which I did not want to do becuase of her involvment with the DPT...

Infection in a diabetic makes their body even more resistant to insulin, this is why their bs usually go up (not always) and why you usually need to give more insulin

Maureen, the people involved in the DPT-1 are not hand picked. It is on a volunteer basis only, however, you have to meet the protocol of the study. We are enrolled and it took 1 year and that is fast. There are many other factors other than being ICA positive. The people in Gainsville could answer all your questions if you are interested.

Laurie, when Jessica is sick and running below 150 in the AM and unable to eat due to nausea/vomiting, we ONLY give the regular dose of Ultrelente...then if needed, we can always chase her with R or H, should she go high during the day. I was told if they are over 150 to add 10% of your total daily dose..I've NEVER done that! Do you increase the boys when they are sick?
Thursday, February 20, 1997 at 21:01:12 (EST) - pdial169.wisedial.wilmington.net

Brenda F.:
Our endo gave us a formula for sick days when they are running high, you take total number doses of insulin for the day (might have to guesstamate) then divide by five and give this # you get every 2-4 hrs. depending how sick they are and how high their bs are

Joyce- I am a physician who has worked in an emergency room for 5 years and a family practicioner for 7. I can respond to your question from my experiences in Ohio. Each state may have different laws but generally they are similar. Under a health care protection law, a physician or other health care provider can report any suspicion of child abuse even for neglecting what may be considered essential health care. They are protected under law unless obviously negligent in reporting. It has to be followed up by a social worker to investigate. But only if they find out about what's being done.

Maureen, I recently saw that product at the drug store. Did you happen to taste it and was your daughter receptive about drinking it. That might be something to keep on hand.

Lisa:
Melissa, that is why we ended up at the emergency room last year. However, we did not go to the Children's Hospital in the city but to our local hospital. I don't think I would do that again since the ER doc didn't seem to know too much about JD.
I was happy that Kait wasn't put on an IV, and got through it OK. Later my endo said that they would have been much more proactive and would have put her on an IV.

Nancy, yes I think we have been through the staging process... Erin only has to have the last IGVT done.... only if her last IAC also is postive (I think this is where she is at... many things in our family to keeptrack of.. hahha)

This is straying a bit but Nicole had the flu this weekend. (Thanks Lyn for the support if you are lurking!) We used Boost (a meal replacement shake), because she couldn't eat solid food. After about 24 hours she was at least keeping the boost down, and it helped us through a tough one. It contains about 36 grams of carbohydrate.

Jimbo, the DPT is a nation (US) wide study.. it is the Diabetes Prevention Trials, it is open to sibling, parents and first cousins of junvenile diabetics... the purpose is to identify those with early (no physical symptoms) of diabetes and try to stop them from developing the disease.. this is quite a simple explanation..

Joyce - the IV steroids really increased her blood sugars. We were sent home on inhaled steroids, and the doc said these shouldn't really affect her sugars, but it was two weeks before she was back to normal. The combination asthma/diabetes is no fun!

Lynne S.:
Hi. Couldn't resist putting my 2cents in. If your child can swallow soft gels there is a form of sudafed or other nasal decongestants which have the expectorant in them. This is the same ingredient as Robitussin DM.

Nancy :
Arlene, what we use for our son is Scot-Tussin Expectorant - clear cough formula - Sugar Free, Sodium Free and Dye Free. I fell into a great deal and the store was clearing them out so I got each bottle for $1.00 with an expiration date of 1/99 - a real bargain. It usually is anywhere from about $4.00 to $6.00. It works great!

Arelene, Sorbutusess Syrup, Toclonol Expectorant are two "cough" medicines, neither taste good or go down easily, but do work. Usually, we end up using Robitussin.
