We've been so busy this fall with soccer and I've been struggling so much with schedules and blood sugars. So we got together on October 20, 1999 to talk about how sports and exercise effect diabetes. Delaine M. Wright, MS, CDE, a Clinical Exercise Physiologist & Certified Diabetes Educator joined us to answer questions about how our bodies react to exercise and what it does to blood sugars whether you're on shots, the pump or pills. Thanks for the welcome, everyone! ... Re: pumps and basketball. The best advice I can give almost everyone regarding exercise is to be diligent about testing pre and post... make notes, know that exercise "type" can cause rather wide variations in BS response (and we can get into that in some detail).... you will most likely have to disconnect during basketball (contact sport issues) - most pumpers find they do need to replace the basal.... better to err on the side of caution by taking half & correcting as needed - watching for patterns the entire time.

Most pumpers will need to decrease basals by half or more with mild to moderate intensity "aerobic" exercise..... but this CAN differ greatly from one person to another... and again, the best thing you can do is to figure out what your "typical" exercise session does to your sugars.... tweaking as you go along from there.

People on MDI (injections) may do it a couple of different ways... by decreasing short acting insulin dosages pre-exercise if appropriate, or probably most easily by increasing the amount of carb ingested prior to exercise. I do work in a cardipulmonary rehab and wellness program with all types of diabetics... pump users, those on injections, and Type 2's managing on orals and/or diet and exercise alone.

when you disconnect the pump for swimming and sports carol you may first need to bolus and amount to cover the basal that you will be missing. Understand or an i confusing you?

Exercise can certainly cause decreases in blood sugar sometimes up to 2-4 hrs after a session. It may vary a bit depending on the activity and the intensity, but metabolism can stay elevated that long.... unfortunately there aren't really any hard and fast answers, but that's a good estimate. hmmmm . . .my worst experience. I've been pumping about six weeks now and fortunately have not had any major problems (no clogged infusion sets or anything. I had some problems with the tape sticking, which took some creative problem solving (and a pair of scissors :) Sometimes the stickiness from the tape is hard to get off. Also, I've had a couple of sites that have hit a blood vessel and bled when I took them out. Probably the worst so far was a night when I was sick and had to stay up most of the night figuring out how to get my blood sugars down -- went through two site changes, a new bottle of insulin, and lots of bg tests. Which reminds me of something: pumping is definately way better than injections, but you can still have high numbers and you need to be more diligent about figuring out why you have them.

I know that Stephanie needs 1 extra serving of carbs to ride for 1 hour and she also found out that if gym is extra hard she will need 1 extra serving. But I just don't understand about bolus and basal yet. She does great on the N's and R's. I felt until she could regulate her own insulin she should not get the pump.

Exercise in Alex can affect him up to 12 hours later, have to be careful of nighttime lows with strenuous exercise during the day

I notice when Kristina is at a great number during a soccer game, that she will drop during the game unless I give her a snack beforehand - about 30 carbs! But if she's in the 200's or 300's during a game, she doesn't drop till later! Is that strange??

Often times people with Type 2 diabetes using orals especially, will notice a RISE in BS with exercise. Anyone ever notice such a thing?? It sounds crazy, but there is a very good physiologic reason for it. And, yes YES - please do stay... this is certainly NOT limited just to pumpers. My apologies if I made it appear that way. ;)

It's really a challenge to know how Marisa's (9) BG's will respond when she plays volleyball. During matches she disconnects for sometimes up to an hour and that seems to work best. I never replace her missed basals-- doesn't seem necessary.

Carol-- to be honest it is much easier for me to deal with only using one kind of insulin (i use humalog but you can also use regular) in a pump as opposed to NPH and humalog on injections. I don't need to worry about peaking NPH and all the problems that go along with that. But then, I was already regulating my own doses before. Is Stephanie calculating her own dosages based on carbs or is she on fixed doses?

She stays on her calorie diet and calculates her own dosages with me making sure I feel the calculating was correct. Her diabetic dr. feels he would like her to use humalog before she starts pump.

I have only been diabetic for about 5 months, so injections AND pumping are both new to me . . .I was only on injections for three months before getting a pump (lucky me!!) and found both to be easy but pumping to be less of a hassle. It is much easier to test bg and push a button than test bg and whip out a syringe. The first time I changed my pump infusion set it took me 45 minutes. After 6 weeks I can do it in 10 minutes, but like with anything there is definately a learning curve. It helps to read alot first. Taking classes is a great idea :)

Soccer, basketball and such sports can actually cause rises in BS's (especially noted on a pump because of the small amounts of insulin that we are typically dealing with (basals) - exercises that are considered "anaerobic" or power-type exercises, involving lots of sprinting for example actually cause the body to pull from stored glucose sources like from muscle and liver.

And yes... I have to agree that with injections, and NPH in particular it seems, the effects of exercise can carry over much farther than a few hrs.... when I was on injections as well, my greatest problems with hypoglycemia were seemingly always related to NPH. It is most probable that we change absorption rates of NPH for hours after an exercise bout.... hence the difficulties.

Carol: I can't really recommend whether or not you should switch to humalog before starting a pump or not, as I have never used regular . . Maybe Delaine could answer that one?? question: are you counting just calories or carbs too? Also, it's definately good that Stephanie calculates her own doses. There is nothing wrong with you checking even if she is pumping.

Counting carbs works out the best. That was the way we were taught. The calories take care of themselves. Its the fat that she has been watching lately

Elevated blood sugars due to "insulin insufficiency" (not enough around) causes the body to mistakenly think that there is not enough glucose around to provide the needed energy. Your body then tries to assure that you will have enough glucose by pulling from stored sources in your liver and muscle. You actually carry quite a bit of sugar around!

I was 315 lbs 6 weeks ago...am now 279...i am type 2..my back and leg is very sore when i try and walk...is there any other exercise program you may reccommend. I am on Glucophage twice/day...Glucotrol....and avandia 4mg. my "normal" seems to be around 140...but today it went down to 96. Is this normal?

Carol Horseback riding and hiking she should be able to leave it on, probably no need to eat extra, she can reduce her rates or suspend the pump, but she will have to test often to see what is right for her.

The ADA guidelines state "no exercise if over 300" or great than 250 fasting with ketones. Do I always follow those guidelines?? No... I consider whether the "300" is a post-meal sugar... what was consumed most recently, when was the last insulin injection taken, when will it peak. I also have patients who use a sliding scale and will correct for BS's this high prior to exercise - although I caution you to correct with HALF or less of your usual correction factor prior to exercise to avoid a low.

OHHHH that's good Delaine --- that was my next question--- how much of a correction should I give if she's too high to play but has to anyway!! I usually give 1 unit of H for every 100 points she's high! So if I give 1 for every 200, she'll be ok ---

I have horses (2 - my "kids"! *grin*) ... (hi Ry!) ... and I ride with my pump on without a problem. Interestingly, for me riding actually raises my blood sugar... I found this out through extra testing and patterning. I now use a higher temp basal during a ride. I also hike.. and rock climb with the pump on. For those activities I reduce my rates. On injections I would need to eat 15 - 30grams of CHO prior to avoid lows.

No, its just a change. It took us several years of constant changing insulin dosages, her hormones went crazy. I don't want to go through the fustration of not being able to control what the body does. She sometimes went to hi or to low for no reason. It's not like that anymore. But it was very fustration figuring out how much and what kind of activity changed her blood sugars.

Carol -- my mother is actually much more comfortable with me having a pump than when I was on injections. I had many problems with lows due to peaking NPH and not being able to eat very conveniently when I "should" have. Now she can feel more confident that I won't have problems like that very often. I have had a couple of lows after meals from overestimating carbs in food at restaurants and things like that.

Greg... what is it that is going on with your back and legs... is it muscular?? You may do well with a non-weight bearing activity, like swimming, or perhaps even biking - a recumbent bike is usually a good piece of equipment in these cases... but it is a little difficult to say, especially as I am not sure what is going on in your situation.

Delaine, I have degenerative disc disease in L 3-4-5 and my legs go numb as the nerves are caught between the discs. It's sometimes very painful. Also have the same condition in C 2 and 3. The bike is a good suggestion. I'll try it. Any other advice?

So far from everyone here and at her camp, everyone on it loves it and is very encouraging. The only other concern is whether it makes it so much more appealing to overeat because you can just correct your insulin now. At first that was her concern. And also if you get sick vomiting, and diareah what do you do not eat, take the pump off what?

Melissa, I agree.. I would start with half the correction dosage during the game. You can always increase from there if needed. Even small dosages of insulin, as are possible on a pump, will assist in avoiding a rising blood sugar.

Alex i think actually eats less now that he is not feeding the insulin. He no longer eats snacks at school. Sick days are much easier because you can reduce or raise basal rates accordingly, no need to ever take the pump off(except for contact sports or swimming) if needed you can suspend the pump from delivering any insulin and since there is no long acting insulin there will be none in her system, but again testing often and checking ketones is key

Kristina's blood sugar drops drastically and VERY quickly when she's been startled or terribly upset by something! Last time we went to see the doc, the nurse decided to give her a flu shot while we were there! Since we didn't know this was going to happen, Kristina got VERY upset till we decided to wait on it! 1/2 an hour later, her blood sugar was 56! yikes!

Greg.. additional suggestions - keep an eye on your numbers and don't be surprised if with your consistent exercise and the weight loss, that your sugars get even lower. You may be able to decrease your medication dosages/therapy with your physician's assistance, of course. Best of luck to you with your program.

Carol--it is much easier to be sick with a pump--as I've recently found out :) no need to worry so much about making sure you get food down, etc. Also, one thing my endo mentioned to me if you do need to suspend the pump, make sure you check blood sugar often at least the first few times, to see how long you can go without insulin. the reason is not only because of high bgs or ketones, but because if you wait until your bg starts to rise before reconnecting, it will take another half hour at least (or so) for your new basal insulin to start working, during which time you can go alot higher.

thats good to know, Last year was a bad one. She was sick alot and went into the hospital 2x so sick that she was in intensive care once. For no apparent reason except for maybe a virus. She got high ketones and even with the help from the drs. on an hourly basis she didn't get better until she got the drip.

Kristina's soccer games last 1 hour and she plays at least 3/4 of it, if not the whole hour! At least 1/2 the game, she's running hard! I usually test her before, at half time and afterwards! But she has a snack after a game that one of the parents brought so I don't know how accurate the after game test is since she beats me to the snacks before I can test her! LOL

OH yea.. games are VERY different than practices! Kristina rarely has trouble at practices! She has a snack when she gets home and then we run out to the soccer field! Leann.. if at all possible, I would have him check every 30 minutes.... the more info you have to go on, the better. Try not to correct the first time, unless you see things going WAY out of range.... attempt to "pattern" what is going on. Basketball is a fairly intense(!) sport, to say the least. Have your carbs handy, and also be prepared to replace that missed basal (maybe by just half to start).... I can't say for sure if basketball will drop him right away if he is disconnected.... I would suggest waiting for a BS rise to begin before I bolused anything.

I'm sure they are starved. They eat lunch at 11:45, and don't eat supper until around 5 or 5:30. Most normal people would be starved. She likes to stop on her way home at the store with her friends and get a snack and a pop. And if what I've learned here tonight she can still do that. We try to keep everyday as close to the same as possible, as far as food intake and times eating. Does that help?

yes Carol that helps on injections but she will no longer need to do that on the pump(except maybe when figuring out basal rates in beginning), she can stop for that snack and pick out exactly what her friends are having, even a slurpee!

How quickly dinner works depends on the dinner... (a higher fat and protein ratio will slow down those carbs) but if he is still eating, you can be fairly sure that your test is fairly accurate. If fast acting carbs take 10 - 15 min to reach the blood stream, then the beginnings of dinner will take a bit longer. Unless of course he starts with dessert! *LOL*

In her school system she has to go to the nurses office because of the blood, If any blood is sited in a classroom, it has to be cleared out and the area surrounding the blood cleaned and sprayed with something. When she was in grade school she was allowed to do it in the classroom, all the kids thought it was neat, and when it was time for her snack or blood test they wouldn't let her forget. BOY high school is really different. Her nurses at school are well educated in diabetes and will listen to me and her. They always call if her sugars are low or high, and check to see if what she did was okay. Usually it was.

The pump is definitely well designed for active lifestyles... and although it may take a bit of a time committment to work it all out, life becomes much more flexible and closer to "normal". :) It is closer to what the normal pancreas would do... although, without the nifty blue color. *wink*

Carol--one of the reasons I got a pump was because I hated the schedules, plus it was dangerous when I couldn't stick to them! I never eat on a schedule now--high school is not conducive to them!! My lunch time varies from 10:45 to 12:15 on a daily basis.

Everyone we know, know that we are very strict on the time schelule, and we try to schedule out activities around the food time. And if now, we take our food.
Both our kids do not like sports except hunting, fishing, hiking, horseback riding, and they do piano, flute and clairnet. talk on phone etc. I would suspect that team sports are sosoooo hard, because you can't pick the time. Hats off to those parents and children who can.

LOL Leann, I WISH we had the pump! In fact, our doc says when we go on the pump we'll have to be on an even MORE rigid schedule!! He says it's alot harder than shots and will take ALOT of effort! WE're still trying to get it.. he's considering it and is just waiting for his "team" to agree to it too! I'm not sure which of the "team" he means cause everyone we've seen and talked to said they'd do whatever he said to! We've been trying for 2 years to get a pump!! We're still hoping! He was supposed to talk to his team again this week but even if they said yes, we have some work to do! He wants her blood sugars straightened out completely before we start! And because we've been working so hard on that already, her A1c is down to 7.4!!! She hasn't been that low since she was 18 months! So I don't know what we're waiting for now... except.... "the team"!!

oh sorry Melissa thought she had her pump already. For the first 5 days Alex ate the same exact meals with no snacks in order to set basal rates but after that he was allowed to be as flexible as he wants with eating(while of course still eating healthy)

I'm so happy with our dr. he waited until Stephanie said she was ready, she had lots of pressure for the last 2yrs. but was afraid that she would sneak to much food and gain weight. She is really trying to keep that under control, and is doing really good with it her last A1c was 7.9 which was down fron 8.5 then 8.9
She is doing great in 1 year. I feel she realizes that what she does effects her life and nothing I do for her can help she has to be the one who does it. I AM SO HAPPY she is growing mentally up.

Go ahead! I see you've been helping folks again with their insulin ratios! Did I tell you Kristina's A1c is down to 7.4 now? LOL and those afternoon lows WAS from the U! The doc reduced it considerably!! LOL

You're definately right Carol . . .sounds like Stephanie (and you) are both ready for a pump . . it does take some work but it really does make things so much better. In terms of control and just enjoying life more!!

This chat forum has been very informative and helped me so much. Tomorrow I'm going to let Stephanie read all that was written, I think that listening to people that has the pump will encourage her, it has sure encouraged and enlightened me THANKS TO EVERYONE. Wow John! All we have is soccer!! LOL But practice is 1 1/2 hours and then we have to wait around another hour for Kristina's brother to get done with HIS soccer practice! I'm about ready to set up the tent and a barbeque at the soccer fields! LOL

Carol, Bruce is an old friend and long time chat buddy who's daughter was on U and H and is now on the pump! Bruce is VERY knowledgable about diabetes and loves to help people figure out why they're going too high or too low or whatever!
